New Patient Information
If you are interested in becoming a new patient, please call our New Patient Coordinator at (508) 830-1201 during office hours. If you get the answering machine, it means Heather is with a patient and cannot answer the phone, please leave a detailed message stating your name, phone number, and the name of your practitioner.
We make every effort to reach new patients as soon as possible. If you have not received a reply within 48 hours, please try again.
In order to better serve you, below is a checklist of things to prepare before your first visit.
- Download all of your New Patient Forms. Fill out and sign all forms before your appointment. Please bring all forms completed and signed for your first appointment.
- Confirm with your insurance about coverage for your appointment.
- If you are coming for a non-covered service, please be sure to bring payment in the form of Visa, Mastercard, Check or Cash. Payment is expected at the time service is rendered.

New Patient Forms
Please download all the forms for your appointment, complete them legibly and bring with you to your first appointment. Don’t forget to include a list of all current medications and nutritional supplements. Some of these forms are .pdf files that require Adobe Acrobat to open.
If you are not able to print out the form, please come to your appointment 20 minutes early and you can fill one out in the waiting area.
